1. Assumption of Risk
I understand that participation in golf simulator activities at Defense Depot,
including use of Trackman Golf Simulators, involves inherent risks. These risks
include, but are not limited to:
- Injury from swinging golf clubs
- Being struck by golf balls or equipment
- Slips, trips, or falls
- Damage to personal property
I voluntarily assume all risks associated with participation in these activities.
2. Use of Equipment
I agree to:
- Use all equipment properly and as intended
- Follow all instructions provided by Defense Depot staff
- Only swing clubs within designated hitting areas
- Ensure that no person is within swing range before taking a shot
I understand that misuse of equipment may result in removal from the facility
without refund.
3. Responsibility for Damages
I accept responsibility for any damage caused by myself or members of my party
to:
- Trackman simulator equipment
- Screens, projectors, or sensors
- Clubs or facility property
I agree to pay for any damage resulting from negligence or misuse.
4. Release of Liability
I hereby release and hold harmless Illinois Outdoor Sports LLC dba Defense
Depot, its owners, employees, and agents from any and all claims, demands, or
causes of action arising out of or related to:
- Personal injury
- Property damage
- Loss or theft
whether caused by negligence or otherwise, to the fullest extent permitted by law.
5. Rules & Conduct
I agree to:
- Act responsibly and respectfully at all times
- Follow all posted rules and staff instructions
- Not participate under the influence of drugs or excessive alcohol
Defense Depot reserves the right to remove any individual for unsafe or
inappropriate behavior.
6. Minors
Participants under the age of 18 must be supervised by a parent or legal
guardian, who agrees to this waiver on their behalf.
7. Acknowledgment
By signing below, I acknowledge that:
- I have read and understand this waiver
- I voluntarily agree to its terms
- I accept full responsibility for my participation
